Vacination Form

Hi All,

I have been away for a couple of months but now I am back. I am putting together the I-485 package for the AOS. I am not filing the I-765 or the I-131 for my wife Johanna. We just got married last Saturday and it was very beautiful, if you want to look at the pictures they are at peterandleidy.weddings.com.

I have a question about the I-693, do we need to send this in with the package? Because this form seems like it has to be fill out by a doctor. Johanna brought back a form DS-2053 title “U.S. Department of State Vaccination Documentation Worksheet”, which states all her vaccinations by name and dates. Have any of you seen or sent this form.

Peter: Congratulations! Its up to you if you get the civil surgeon to fill out the I693a or take a chance and just send in the DS3025. I just sent in the DS3025 as I was willing to take an Rfe or a delay of a couple monehs (if asked for at interview.) No one asked for it and we were OK without it but some people have been RFEd for it. So you can decide if you want to chance it or not. Good Luck

I sent that form. It worked fine. They even looked at it during the formal interview. It is just as good as the 693....good luck.
