Hello again everyone!

The hubby and I are currently working on getting our AOS forms straight and we have a quick question.

The hubby was born with two last names hyphenated. (his mother's last name and father's last name as his parents weren't married...which is what's on his birth certificate) He went to a solicitor/lawyer to have a form signed saying he was taking only one of the names from now on (he dropped the second name) and that's how he was able to get his passport originally.

Now when filing for AOS, do we need to submit a copy of that lawyer's form with our AOS package? (I'm assuming so, as his birth certificate will say a different name than his passport and our paperwork, etc) Or is there a form specifically for this? (I've looked around and haven't seen one but maybe I missed it?)

But just wanted to make sure. Anyone else have this situation with a name change?

I added the appropriate document to show a change of name when I submitted mine. Adding it can't hurt at all and intact will help the officer make sense of all the names flying around and see that they are legal. Good luck!

N-400 Journey

June 9th 2012 - Application sent

June 14th 2012 - NOA sent

June 28th - Biometrics letter sent -- July 26th Appointment date

July 12th 2012 - Early Biometrics/Walk-in

August ( ) - Interview letter received -- September 14th Interview date

September 14th 2012 - Interview! Oath date in hand, September 20th @ noon!
