like my married name is Vanessa dos Anjos Phoenix and my single name is Vanessa Santos dos Anjos.

So I need to fill the form as already married right ?

and in the line that they ask about my husband name I must write his First name or his Last name ?

Your husband's full name. You can continue to use your maiden name if you want, as some women do, but it's easier all around if you write in your full married name.

thanks for ur help ..... so I will use my married name ... cuz my passport has it .... and all my docs now are with married name :)

and about the line that I must write about his infos .... I must write his first name or his last name ....

Cause I was looking on the examples forms from g 325a form is showing the last name not the first name on in it .

You have to use your maiden name (your name before marriage as it is in the birth certificate) and they specified that in the form itself...
