Hi guys

We are gatherings documents for my AOS and i have question regarding the medical. What shots/vaccines do i need and must i go to Civil surgeon designated by USE or i can go to any doctors here?

Any reply will be appreciated. Thank you!

Timeline

The I-693 form, whether you have the full form filled out or just the "supplemental", needs to be filled out and signed by a USCIS-approved surgeon. Just as an FYI, they will not bill health insurance for this service; it's paid all out-of-pocket.

However, they will accept proof of immunization from other doctors. If you bring the vaccination records from other offices, they will transcribe it to the vaccination record page.

I can't remember which vaccines are required, but some are age-dependent. (The HPV vaccination, I believe, is only required by USCIS for women up to age 26.) I'm sure someone else around here will have that information. (Or try googling it.)

Mine, i continued my lacking vaccines but when i went to the surgeon she need my HIV and blood test but i dnt have the record cause i gave it to the POA. So she redone all my medical including HIV test and TB test, and everything gosh. very expensive though hehhehe...and i also sent my old vaccines record together the medical result from my surgeon. thanks God were done.

The civil surgeon didn't "need" anything. They lied to you so that you would pay them for a full exam. Sorry, but you were tricked.

For a K-1 who had an exam less than 1 year ago, you need to bring your vaccination recortds to the USCIS-approved Civil Surgeon and they need to fill out the vaccination section (part 2.5) and sign part 5. THAT'S IT!!!!

You may be able to get the needed vaccines from your regular doctor using your insurance. All insurance companies cover "preventative care" so the doctor should be able to give you the shots you need, but many doctors say "NO" when they hear that it's for immigration. They can charge you more money than insurance pays them. Don't say it's for immigration. After they give you the vaccines, they should give you paperwork to take to the Civil Surgeon to prove that you received the vaccines. The CS will then fill out the I-693 form to send in with your AOS.

Ahh, thanks for putting that because I thought it while I was typing, but went back to correct some grammar mistakes and then forgot to add that into my post.

I had said that the civil surgeon will accept vaccination records from other doctors. I failed to add that it meant you can go to your other doctors and get an immunization under your health plan, and then take the paperwork back to the civil surgeon.

Yes, you HAVE TO go the the Civil Surgeon to do your medical exam. I called tons of them to check the price, and choose to cheapest one. It ranges from $250-$500.

Bring whatever vaccine records you have to the doctor, so they can take a look and decide that they need extra. I paid around $250 total, which included blood test and 1 shot. TB test needs to be done, but I went to the Dept of Health for the free one, then take the records back to the civil surgeon for the report.

I thought that i will have another vaccinations in here but the assistant of the USCIS Surgeon told my husband that what we need is the i-693 supplemental form,i handed my vaccination sheet from the Philippines and she just asked my id and paid $50 for that and gave the sealed form as well as the copy of it.

There is a new form I-693 that includes a section for the immunization. The "supplemental" form is no longer accepted for medicals given after June 1, so your Civil Surgeon is not familiar with the new rules. See this whole page http://www.uscis.gov/portal/site/uscis/men...0004718190aRCRD
