Filing for k-3 and k-4 visas

Can anyone tell me step by step on how to file for my wife's visa and her child's visa? If possible please be specific give all the details you can.Also I have added a few questions that me and my wife have cause I'm not really sure the proper way to do this.

1.When filing the petition, I know I will include her child on hers but do I need to file one separately for our daughter as well? Will there be one fee of $355 for both or double? Do I send the payment of $355 when I send in the I-130 and where will this be sent to?

2.Is this going to take us a long time to get both visas? Will she have to be issued hers first then we wait on her daughters? Is it possible for them to obtain their visas at the same time?

3.Also I was told that we would have to have permission from the child's biological father for her to be able to leave the country to come to the U.S. Is this true? Cause my wife has no idea where the father is or his family, the father has not been in the child's life at all, she only knows me as her father.

2) Either... You can have the K-4 issued concurrently or utilize follow-to-join.... a good read of the guides will help you understand your options

3) In most places the answer is yes, however, each local jurisdiction may have a judicial or some other sort of remedy available for people in your fiancee's situation. So a check with the USE Manila website or others who have come from there, they will be a valuable resource.
