In process of filling out AOS forms...ran into a snag

So we didn't get any medical report from the embassy/medical...

we were filling out AOS papers yesterday & realized the 693a supplement form requirement. Then found out about the civil surgeon thing. We couldn't find a civil surgeon to fill out the form, unless we did the MEDICAL again. so we set up for an appointment for that...she said the exam is just $75 (a steal compared to the $225+ ones). It's on friday. we got the price list today & it's scaring the ####### out of me. its saying how the AIDS test, TB test, etc is REQUIRED TO BE DONE....which is ALL extra $$$. I am afraid they wont take our word for it, when we say he did all of this already...but no papers to prove. he only has his vaccination records.

Has anyone ran into this issue?

I have actually heard of people on here who HAVE had the civil surgeon take their word for it. I hope you have one of those.

Did he happen to have them mark his vaccination record when he went to the medical? I hope so.

Did he happen to have them mark his vaccination record when he went to the medical? I hope so.

My luck with doctors when it comes to shots & stuff has always been good...but don't know what to expect of this one. Gonna keep cross my fingers. if i get a stubborn one...thank god for credit cards. :crying:

Yeah, they gave him a printed sheet with all this vaccinations & he also got some little colorful booklet thingie with the same info. :wacko:

That is great news. That isn't his "word", that's actual proof. Make sure he takes it with him.

The thing is, you are kind of stuck at this point. He has to have that stuff to continue on in this process. If you have to suck it up, you have to.

Well.. you dont need to have a Medical Examination again, since you already had it. the medical that you had will expire after 1 year. I just got a vaccine record with me too, and no medical report data. Just tell the civil surgeon that you already had your medical exam from your country and you want your Vaccination record tranferred on the I-693 form. The civil surgeon will look at your vaccination record and if you lack some of the vaccination, He will give it to you. and this would be another fee.

I payed $65 for the I-693 form and $75 for Hepa B vaccination.

You need to fill up the part 1 of I-693 but dont sign it until the surgeon will ask you to do so. The civil surgeon will fill in the rest of the form. and the form will be given back to you in a sealed envelope.

Nov. 3, 2008 - Packet delivered, 6:56 am, Chicago, IL 60680

Nov.12, 2008 - Check cashed

Nov.14, 2008 - NOA1 for I-485, I-765, I-131 received

Nov.24, 2008 - I-485 transferred from Chicago Service Center to California Service Center

Dec. 5, 2008 - Biometrics Appointment -- done

Dec. 8, 2008 - I-765 Touched

Dec. 9, 2008 - I-485 Touched

Jan. 12, 2009 - EAD card and Advance Parole Received

Jan. 13, 2009 - Applied for Driver's Instruction Permit

Jan. 15, 2009 - Driving written test passed

Jan. 26, 2009 - I-485 Touched

Jan. 31, 2009 - Actual Drive test passed "got my Driver's License"

Feb.25, 2009 - Took NCLEX-RN -- PASSED--- Anais,RN

March 9, 2009 - Green Card received

March 31, 2009 - Got a JOB!!! PCU nurse in the making...
