My wife had the Vaccination Supplement filled out by a civil surgeon who charged us 200 bucks. One of the Vaccination she needed (MMR) required a return visit one month later for a second shot. Well, the doctor filled out all the papers before we returned for the second shot and sealed them in an envelop (we have a copy). When we retuned a month latter for the last MMR shot, he wanted another 80 bucks. Then he would not accept our insurance for the shot, even though he accepts that type of insurance. So i went to my regular doctor and got the second shot.

Anyone think this would be a problem if the Vaccination Supplement was already fille out be a civli surgeon, and the last shot was done by an ordinary doctor?

Has this happened to anyone else?

Goodness!!They really got you good. I've done my shot in ordinary doctor and bring to the u.s.c i.s civil surgeon because they also told me that they charge $60/shot. so all we paid is just the I-693 complete exam which is $250 and they just tranferred my shot records to I-693A. so I think it doesn't matter where you get your shot as long as it is done. somebody correct me if I am wrong.

My husband also got the first MMR shot from the civil surgeon, but has not returned for the second shot yet. The civil surgeon said that it wouldn't be a problem as long as the first shot was indicated on the vaccination supplemetal.

You should be just fine.

I just get some of my vaccination here in the US in the ordinary doctor, then i brought my records to the civil surgeon to transfer to Form I-693, i paid $60. I think its ok coz the immigration was already approved my visa.
