hello everyone...

Need some help here pls. I'm doing the AOS, the part of Medical Examination. What form do I exactly need? Since he came here as K-1 visa from the Philippines..we only need the vaccination supplement.....RIGHT? Is the form i'm going to use is I-693A? or can I just use the form that St. Luke Medical Center Form DS-3025?

As long as the K-1 medical exam was less than one year from the AOS application date, you don't need another medical exam.

You will need to have the Vaccination Supplement prepared by a Civil Surgeon here in the U.S. Call around to more than one to find the best price as their prices vary significantly. We paid $35.00 for the Civil Surgeon to transfer the information from the Vaccination Worksheet (from the K-1 medical exam) onto the Vaccination Supplement. There will be an additional charge if any vaccinations are necessary.

Ditto what raymaga said. Just to clarify, bring your vaccination worksheet (that is the DS3025) and the civil surgeon will fill out an I693A (vaccination supplement) for you. Most people pay something between 20 and 60 dollars. If the civil surgeon you call says you must have a full medical, call someone else. The only exception is if your K-1 medical was more than a year from when you're sending in your AOS application.

Some people have sent in just the vaccination worksheet (DS3025) and have never been asked to send in the vaccination supplement. Others have done the same thing and gotten an RFE for it. If you're really short on cash and don't mind taking a risk, you could just send in a copy of the vaccination worksheet and see what happens.

Provided that the DS-3025 shows that all required vaccinations have either been had or are waived as medically inappropriate.

The reason why this is an ambiguous issue is that the vaccinations are not required for a non-immigrant visa, so a K-visa person may not have all the vaccinations on entry and may need to get some in order to adjust status.
