Need Assitance with Supplementary I693A FORM

I am sure this question has been asked several times but i am unable to find the specific answer. I was told that since my wife had already medically examined before the K-1 interview and the Medical packet was taken at POE and theoretically it will be forwarded to the USCIS. Now i am applying for her AOS I-485 Packet. I need to have that Supplementary Vaccination Form Done. I have talked to few doctors in my area and all of them are contradicting with each other. Some are recommending about 10 vaccinations and some are only three as MMR and Td. What is really the requirement? There is nothing i found relevant to that. Thanks in advance for your assistance.

First, there is no more I-693A. That has been eliminated as of May 1, 08. The new revised form I-693 is used. It has a vaccination section which you will need to have completed by a civil surgeon.

Look at the form I-693 (page 3). It lists all the required vaccines. You won't need all of them as some are gender and age-specific.

The I-693 instructions (page 4, section III) tells you exactly what you need to do if you had a physical exam overseas less than 1 year ago. Don't let the CS talk you into having another medical exam. It will cost you a lot and is not necessary.
