Other than not knowing why NVC didn't send them your wife's 864, I'm not sure what your question is. Call NVC and find out if they sent it. Then you'll have to deal with the embassy to make sure they receive(d) it. Since you overnighted it so close to when your expedite was approved it is possible that the embassy was alerted of the expedite before NVC even opened or dealt with the 864. I'm sure they'll forward it but you want to be sure so call.

For the other stuff, since you expedited, they want everything sooner. Just do it like you'd do it if you hadn't expedited, only do it ASAP. Get your medical. Fill out your DS-230 (the Sponsor fills that out) and have all documentation to support it.

Like I said, the time between your expedite and when they received a lot of these docs at NVC was pretty close. So talk with NVC and see what they have or haven't forwarded to the Embassy. The embassy should get your documents from NVC so you want to make sure that NVC has sent them out and if they haven't when they will.
