confused of K1 guidelines number 13 (on how to assembe I-129F package)

13. Proof of Legal Name Change if either the US Citizen and/or foreign fiance(e) is using a name other than that shown on the relevant documents. You must give USCIS copies of the legal documents that made the change, such as a marriage certificate, adoption decree or court order.

My NSO security paper at first, my family name there was with my mothers maiden name lets say "Maria Rizal" since my parents that time i was born was not yet married so i am an illegitimate child. and then right now i have it changed on the NSO paper as my real name using my father's surname "Maria Lazo" so it is being legitimized. i have been using this name "Maria Lazo" since i was a kid. i just found out on the NSO security paper that is it is still Maria Rizal". do u think i have to send or include my BC (the anotated one with the NSO) in the I-129F package or i will just bring those documents during USEM interview. pls. let me know about this. pls. help. :help:

Do you have 2 seperate Birth Cert. from NSO ?

If so do each birth cert. have a different name?

the first NSO paper i got was with my mother's maiden "Maria Rizal" name and right now i have it annotated (the second copy) me to be known as Maria Lazo -father's surname. just the surname though coz i was illegitimate before. though im using my father's surname since in elementary/or when i was a kid.
