How do I get vaccination supplement (I-693A)? When I went to my medical examination in London I was given a “vaccination documentation worksheet.” Is this ok? If not how can I get hold of I-693A? I was not given anything with this number on it as far as I am aware. If I cannot get I-693A what will happen? It also seems that on the vaccination record that the doctor gave me, they made some mistakes with the dates that I received my shots. According this I got an MMR shot in July 2005. This is wrong as I was in America in July 2005. Is this an issue? I did have the vaccine, just not on the date stated on the record. I shouldn’t have to pay for another medical just because a doctor wrote something wrong should I?

To get the I-693A you need to take the worksheet from your medical to a civil surgeon in the US. You can find ones near you here:

Shop around for the best price and make sure they dont try to make you do the full medical again

I cant remember your current status off the top of my head but its on the sample forms on this site.

Your non-immigrant visa number is on your K-1 in your passport, on mine (also from the UK) it is 8 digits on the right hand side and is in red print

Some people file with the forms you already have with no problem. A few get RFE for an 1 693 A vax suppliment. They simply take your DS 3025 and transpose it to another form. We found a doctor here that did it for free so there was no loss except about an hours drive to a really nice city. Many people have problems with civil surgeons that eaither need to make a boat payment, or just don't understand the law. The CS wants to do another medical examination. This happens quite frequently here and should be ignored.

There are also some very rare cases where the USCIS demands another medical although it is clearly against their guidelines. There seems to be no recourse for this condition, but luckily it is rare. Don't fret about all of this and don't let any gung ho CS take a bunch of money from you.

When we submitted the AOS paper work we only included a copy of the DS-3025. When we recieved the interview apointment letter one of the things listed to bring is the I-693 so we have scheduled an apointment for that. I figure its better to be safe then sorry.

Not sure what "Processing sheet for form I-485" is, but if it's what I think it might be its for USCIS internal use and not something for you.

You can describe your status as K1.

For visa number you can use either the Control Number in the upper-right area of the visa or the red number below it in the lower-right area of the visa.

I believe that it does indicate that some of the vaccinations are medically inappropriate. I think there is a suggestion that they should be waived.

We only submitted the DS3025 and as Yodrak pointed out - my wife didn't need any more vaccines; she was eligible for blanket waiver even though her vaccination was "incomplete". We didn't have any problems (we didn't submit I693a etc.) Good Luck

WR-702 "processing sheet for form I-485" used to be a part of the K-1 visa process and is now obsolete.

I called around the places that are around here and the cheapest I was told was $120. One place wanted to give me the full medical again even after I explained that I have already had it. Another place on the list doesn't do the immigration medical stuff any more apparently. i think we are going to file with the other form that I was given at the medical in England and hope for the best.
