NO :no:

You will need to take vaccination record to a US Civil Surgeon for transcription to an I-693.

I-693's directions stipulate that only the vaccinations part is needed to be done for K-Visa holders.

You are filing for AOS so profile should say "Filed for: AOS Pending", and I assume home country is the Philippeans NOT N/A, PLEASE fill in profile details and timeline before posting questions, this helps understand your immigrations status.

The vaccination supplement (I-693A) is discontinued as of May 1, 2008. The new form is I-693 which includes a vaccination section that needs to be completed by a civil surgeon.
