hello can somebody help me to fill-up my Social Security Card?

b-coz im kinda confused to fill-up my forms..... this are the no# of the questioners that im confused to fill up.

Application for a Social Security Card.

3. CITIZENSHIP ----- ( choices )

11. Enter the Social Security number previously assigned to the person listed in item 1. ------- ( choices )

12.

13.

on the 12 and 13 question do i need to put ( NONE ) for the answer?

I hope somebody can help me b-coz im so confused and thanks in advanced....

On the CITIZENSHIP part you have to put your real citizenship unless you're already an american citizen (put american)... About no. 11, you'll just leave it blank or put N/A coz it's your first time applying for an SS ID... I think numbers 12 & 13 is related to number 11 so just leave it blank or put N/A to the space provided...

The answer to #3 depends on your current status. If work authorized go with "legal alien allowed to work."

and the alien will not have an EAD.
