So...after reading it all I'm still conflicted...

I'm getting married on Friday...I want to have everything ready to send in for my FH's AOS for when we come back from Vegas.

Even after reading the pinned thread & PMing with someone who's just been through it and what she recommended, I am still conflicted over whether or not we need to go and see a civil surgeon to transcribe my fiance's DS3025 to the I-693 or whether we can just send in a copy of the DS-3025 with the AOS packet.

I've been procrastinating and holding off on calling the civil surgeons in my area because I am so unsure of what we need to do.

Can anyone please help?

If you received a copy of the Vaccination Documentation Worksheet (Form DS-3025) from your Panel Physician, you may include a copy of that form in your AOS submission in place of Form I-693

This is just my opinion, so take it with a grain of salt. It depends on when your medical was completed as to if you need to do the I-693. My medical was done before the cut off date requiring the I-693, so I could have just sent the DS-3025. I believe the cut off date was May 2008?? but I can't remember, but I'm sure you could find it on USCIS's website. Anyway, even though I was o.k. to just file the DS-3025, I saw several reports of people receiving RFE's for the I-693 even though their medicals were done before the deadline. SO I decided to play it safe and get my DS-3025 transcribed to the I-693 so I could send my AOS package off and not have to worry about it.

The problem is, many civil surgeons down here don't understand that all you need is your DS-3025 transcribed (if you had all your required immunizations done before comming over) . Many civil surgeons will charge 100$ or more and/or insist on performing the medical exam again or they won't give you the papers. IF your imunizations were done before comming over, I say why bother with all that ####### again. There is a doctor in Texas that will transcribe your DS-3025 for 20$ and have your I-693 mailed back to you in a week or less. I and various others have done that on this site and have had no problems or RFE's due to it. You can find the Texas doctor's adress in the pinned thread, it was updated once so look for the most recent one.

Jun 25/09 - Green Card Production ordered - Finally after 10 months!

June 30/09 - Received welcome letter!

July 10/09 - GREENCARD! - All done for a bit now!

May 20/11 - Sent in I-751 for removal of Conditions

Thank you...my fiance's medical was done on June 30th, so I'm guessing we'll have to get it transcribed. I guess I should also get to calling the civil surgeons in my area and see what their deal is. I believe I saw the texas one in the pinned thread so if it doesnt pan out with the ones here in NJ I will be contacting him :)

The instructions in the I-693 are VERY clear. There should be no confusion. Technically, a K-visa holder whose medical exam was completed within 1 year of applying for AOS only needs to have a civil surgeon transcribe their vaccination history onto an I-693 and sign it. That's it.

That's the technical answer, and will result in RFE-less journey towards permanent residency (at least as far as medical goes). But many people have chosen to go the DS-3025 route and have gotten approved. I agree with the previous post about the cutoff date factoring in here. It was May 1, but with the 30 day grace period, it's June 1. Since the medical was after June 1, there is a greater chance of an RFE if the I-693 isn't provided in my opinion. I would check the CS's in your area, and call Dr. Arnold if all else fails. She has a good track record and is used widely by VJ members. Most CS's are white-collar crooks who try to take advantage of unknowing immigrants who trust them without question.

Bottom line is that if you want a worry free journey or need the EAD as soon as possible you need to include the I-693. If you can take a 2-3 week delay, try the DS-3025. It may work, it's totally hit or miss.

Thank you very much for this reply. I will definitely start calling around to the CSes in my area. I guess depending on cost it may even be better to just have Dr Arnold do it anyways, but I'll see.
