Hi,

I am a K1 visa holder and i am now filing my Adjustment of Status (AOS) papers. I am just wondering, do i need to send the original "Vaccination Documentation Worksheet" which i received from St. lukes? or, i will just photocopy it?

No, you keep that vaccination record, you need to get it transferred to an I-693 by a US Civil Surgeon, and send the I-693 to USCIS with AOS.

i agree with the above statement. go to uscis website find the list of civil surgeon they recommended to evaluate your vaccination record in st. lukes. and the surgeon will sealed and sign the form I-693. make sure you will sign it too before they will sealed it. they will stamp the envelop that say: DO NOT OPEN FOR USCIS ONLY.

but, maybe it will also differ in every state.

i've got 3 vaccinations again that time. so, i have 6 vaccinations. 3 from st. lukes, 3 from here.

hello there...

pinay ka naman so tagalugin ko nalang ha... nagfile kami ng hubby ko last august ng aos... puro lahat photocopy lang ang pinadala namin... no need to go to civil surgeon for your medical kasi 1year naman bago magexpire yang medical natin from st. lukes... so good to go na yan basta photocopy mo lang lahat.. like marriage cert, passport, I-94, visa mo... goodluck..

I sent a photocopy of the vaccination form from SLEC together with the sealed envelope from the civil surgeon after the vaccination is done. Before filing for AOS, we actually went to have 3 more vaccinations, 1 for TD & second shots of MMR & varicella coz it is required before filing.
