fax a letter to USE telling them to correct your name, and fax likewise your marriage certificate or any document showing the married/legal name you are using now. address it to Operation Unit Immigrant Visa, Manila USEmbassy. they will correct your file and you should pass all your forms with your legal name written. Indicate your contact numbers and email add in the letter you will fax to them.

Sorry, I cannot find the fax number I used before. Have your petitioner call USEmbassy 301-2000 and ask for the fax number or better yet specific instructions for them to correct your file.

Thanks for replying... I called the USEM call center to schedule for interview and also I asked if what will I do regarding my situation... She simply answered "use my married name" since I'm using it now... When I text delbros, use my married name... When I fill-out the DS forms, use my married name (I remember in DS156 it says that write the name the same as what it is written on the passport, so meaning I should really use my married name)... And when I go to St. Lukes, use my married name...

I will have my interview on November 21...

Thanks jamesNrei... God bless you... all of us!

that's good! I faxed a letter and my birth certificate to correct my name. So during my interview my files were already corrected... So maybe you should do that too, so during your interview all your papers are polished to avoid possible confusions or delay. Good luck on your interview!

Thanks, I didn't expect that I receive the letter today... I was about to go out to call the USEM when the delivery man came...

I called for appointment already and the operator said that would be fine.. I will just use my married name (husband's surname) on all forms (from St. Lukes to USEM)... Besides, they may see my maiden name on the passport, so it's not really a problem... I was just confused when I first received it...

I chose November 21 coz I don't have the marriage index yet (I know this will take about 2 weeks) also the NBI then next week I'll go to St. Lukes...

I'm praying that everything will be fine for us... God bless us Anne and all of us here! :)
