Hi. im waiting for my NOA2 and just doing some research before hand. Does anyone know what a OF-156 form looks like? I found this on the vancouver embassy info of things you need:

* When at the K1 interview, they will request that your documents be in this order:

1. passport

2. photographs (print name on back)

3. OF-156/DS-156

4. DS-156K

5. birth certificate (long form)

6. marriage certificate (if any)

7. termination of previous marriages

8. police certificate(s)

9. court records (if any)

10. Affidavit of Support and signed OF-167 attachment (they will also ask for income tax returns and the W2 as supporting documentation)

11. medical (they do not ask for the chest x-ray so you don't need to bring it with you)
