Picking an advance parole date

Timeline

We're going to be applying for advance parole when we send in all our AOS documentation. We're doing it because my fiance's mother is quite sick, and honestly, there is a very good chance she might pass away or have to be hospitalized in the next two years. But I was looking at the AP form, and it asks for a date you'd be using the AP... we don't know what date that is. We just want to have it in case... any suggestions on what to do there?

Also, I notice in the example forms section there used to be a document on AP additional information or explanation letter -- but that was taken down by request of the author. Anyone have that? Was it helpful?

Statement of circumstances that warrant issuance of advance parole

I wish to apply for advance parole and obtain the necessary permission to travel to my home country prior to receipt of permanent residency status in the event of emergency or family illness. I also have a house for sale in the United Kingdom and will need to return there to complete the sale. I do not have any specific plans to travel at this time.

We did very similar, just saying in case of family emergency and for second wedding ceremony, since our marriage was very short notice and so my UK relatives did not have the chance to attend. We put rough dates, as an estimate of when we'd like to travel. Like the others said, I don't think they really care about the exact dates you'd want to travel.

Removing conditions:

10/9/10 - Package sent to Vermont

10/13/10 - NOA1 date, received 10/18
