Hi, I just need some enlightment here. My medical exam was done lasyt Aug.20, these year and I still need a shot of Td, so my vaccine history is incomplete. I had a copy of my DS-3025 and is it really required to get a shot of td vaccine and let the CS transcribe it to I693? or I can go to a CS and let him transcribe it without receiving the vaccine. Please somebody help me here.. Thanks and God Bless.

He will tell you if you still need to get the Td shot. You can get the Td shot at the public health center for free and they will give you a yellow form and you can give that yellow form to the CS and have it transcribe to the I-693

In my case I went to a certified civil surgeon and they do the rest of the transcription to I-693. However, I was required to get another shot of MMR & varicella plus the TD shot. To my knowledge, if you have incomplete vaccine history they won't transcribe it.

Typical CS crook. They know this is something we must do, so they try to gouge us. Look for another CS or call Dr. Arnold. But you should get the Td vaccine first because the CS will charge more for it. You can get one at a health clinic or using insurance (sometimes). Then take the evidence to the CS to transcribe it.
