From the FAQs Page (AOS)

Q:Will I need to take another medical exam since the medical report is only good for one year?

A:In most cases, no. The results of your K1/K2 medical were in the "sealed envelope" that you brought with you from your home country. The I-485 instructions mention that no additional medical is required if filing for AOS within a year of the medical required to obtain the K1/K2. You should simply send in a copy of the I693 (sheet that lists your vaccinations) that your doctor should have provided overseas when you had your medical. And...according to a newsgroup posting...."Went up to the local USCIS (INS) office in Omaha today and they told me that the letter saying "Adjustment of Status / I130 Interview" is just a form letter they send out whether you've applied under a I485 OR a I130.... They also told me that, despite what the letter says, I do NOT need to get another medical done even though its been more than 12 months since my K1 medical at the London Embassy."

did you send in an I-693 with completed vaccinations and signed by a Civil Surgeon?

Read the instructions on the i-693, they are very clear. Section III on page 4.

Did you send an I-693 vaccination form with the I-485 to adjust status?

If you did not, you may get an RFE for it or the interview letter may request one for the interview.

That was not a "civil surgeon", that was a panel physician that works with the embassy for immigration purposes. There are no civil surgeons outside the US.

Now, that you are in the US, you must get a civil surgeon to get them to complete and sign I-693 for you. Some people have just sent in a copy of the DS-3025 "vaccination worksheet" instead of te I-693 and have gotten approved, but it's a risk of getting RFE'd.

no i didnt send in the I-693, when i filed for the AOS - i did get a rfe but it wasnt for that it was for tax stuff that they wanted ...

the interview letter just says: a completed medical examination form I-693 and vaccination supplement in a sealed envelope unless already submitted ...

i know i sent them the vaccination form before and they have it on file .....

so tomorrow i'll call a Civil surgeon and get an appt ....

*sigh*
