Affidatives sworn from third parties

I've read a lot of topics about this but still have questions. for I 130 to prove the bona fide of marriage we are submitting 2 affidatives statement that are notarized. My question is this affidative have to be in any kind of form or can it be just like a simple letter?Do my friends have to put my A# and who do they address it to? can they jsut put "to whom it may concern"???any ideas???

Another question is while submiting my I-485 I need to attach the petition I130. Do I attached the whole package including these affidaties and other supporting togethers or just the form???

Any help will be greatly appreciated...
