MY DS-230 CHECKLIST

Here is my DS-230 checklist:

1. NSO birth certificate plus photocopy

2. NSO marriage certificate plus photocopy

3. Photocopy of biographic page of passport

4. NBI clearances in married and maiden name and photocopies

5. Police clearances in married and maiden name (is this really needed) and photocopies

6. 2pcs 2x2 visa photos

7. DS-230 form, Part I signed

My 5-year-old daughter's checklist:

1. NSO birth certificate plus photocopy

2. Our marriage certificate plus photocopy (to show that we were married before her 18th bday)

3. Photocopy of biographic page of passport

4. 2 pcs 2x2 visa photos

5. DS-230 form, Part I signed by me

6. Optional (?) Affidavit of Illegitimacy

Did I miss anything? Also, on my daughter's form #20 states to list all places lived in since age 16. Do I need to fill this up for her?

I also read on a wiki page that I should not leave any gap in my employment history. I was in between work for 8 months from 2003 to 2004 and i filled it with "mainly relieving other doctors at their clinics when they could not come in". Is this ok?

Will be sending all the form out in the next two days. would LOVE to know your responses.

Spetember 19,2008 - Document Verification done -sent back to US Embassy Manila

November 03, 2008 - Case under review

November 26, 2008 - VISA printed

November 28, 2008 - VISA in transit

December 02, 2008- VISA IN HAND

January 12, 2009 - Arrived USA, POE Los Angeles

January 21, 2009 - Got married

January 22, 2009 - Applied for SSN

March 17, 2009 - Biometrics Appointment

March 21, 2009 - SSN card arrived in the mail

April 6, 2009 - took driver's license exam and passed! (written and road test)

April 10, 2009 - Repeat Biometrics Appointment

April 14,2009 - Received AP documents in the mail

April 16, 2009 - Received EAD in the mail

SEptember 4, 2009 - GREENCARD received

No need if you didn't spend more than 6 months in another country since you were 16.

I was in between work for 8 months from 2003 to 2004 and i filled it with "mainly relieving other ok?doctors at their clinics when they could not come in".

I'd just say unemployed for that period.

Did I miss anything? Also, on my daughter's form #20 states to list all places lived in since age 16. Do I need to fill this up for her?

No need.

K1 denied, K3/K4, CR-1/CR-2, AOS, ROC, Adoption, US citizenship and dual citizenship

NBI? Married name and maiden name? You should pass the married name.

Police clearance? No need to pass it if you are not stay for more than 6 mos to the other country..

Photocopy? No need to pass it to the NVC..

form#20 just state your address in the Philippines

NBI? Married name and maiden name? You should pass the married name.

Police clearance? No need to pass it if you are not stay for more than 6 mos to the other country..

Photocopy? No need to pass it to the NVC..

So I can put our current address on my DAUGHTER's #20? As for the NBI, they said in all names used, including maiden and married name. So that was what I did.

NBI? Married name and maiden name? You should pass the married name.

yes, put your current address.. NBI, i only pass my married name.. coz when i changed my status, my name appear in the NBI was my SURNAME de MARRIED NAME, GIVEN NAME y MIDDLE NAME.. so, when you observed it.. the MAiden name and married name was appear.

For the employment--> just put there "Physician Reliever" for those year/months (I was also a physician reliever in the early part of my employment history after board exam). No need to send photocopies, just send the original copy you got from NSO and NBI. For the NBI, just get a new copy few weeks before your interview and also make sure you have extra copies of your documents (NSO, etc) for your interview as well and for future use. You are well prepared.

For your daughter just write your current address (if she's less than 16). Good Luck and God Bless.

27 April 2010 - received I-797C/ Request to Appear for Naturalization Interview

17 June 2010 - Oath Taking (Memorial Auditorium, Sacramento, CA)

I only submitted NBI clearance in my married name (no need for the maiden name since we have changed our status) and I also submitted Police Certificate (not Police Clearance) from places I lived since I was 16. For the employment--> just put there "Physician Reliever" for those year/months (I was also a physician reliever in the early part of my employment history after board exam). No need to send photocopies, just send the original copy you got from NSO and NBI. For the NBI, just get a new copy few weeks before your interview and also make sure you have extra copies of your documents (NSO, etc) for your interview as well and for future use. You are well prepared.

When we get an NBI clearance, we are given two copies automatically, one personal copy and one copy for whatever purpose we need it for, in this case the DS230. So the personal copy is also an original and is valid for the same time period the other copy is which is one year. Also, did "no record on file" appear in your married-name NBI? I was told that this is because this is a new application and obviously they have no record on file.

I included photocopies of the documents as per instructions on the site I was directed to when I downloaded the coverpage for the ds230. As for the CENOMAR, the only surviving copy is with my husband and he will attach it to the form when i send it to him.

Now regarding the "physician reliever" thingy...will they not ask for references for this?

I am preparing the same for my wife in the Philippines and may I ask what is CENOMAR? Glad I read this as I think she only has NBI clearance for her maiden name as needed to marry. So I will pass this information on to her. Thanks friends

10/29/2008- NVC approved DS-230, Called today 11/1/2008 from Phils to ask status and was told the good news!!!

11/06/2008- Interview date set 12/12/2008, Received email reply confirming date but still need letter!

11/20/2008- Letter received in Philippines

11/25/2008- Medical at St Lukes everything ok!!!!!!

12/12/2008- Interview complete but need Marriage Index from NSO. Applied for 12/15/2008

01/06/2009- Got Marriage Index at NSO & took it to USEM, was told they will call when Visa can be picked up

01/16/2009- Visa delivered by Delbros....fantastic!!!!

CENOMAR is CErtificate of NO MARriage. You are supposed to have obtained this prior to getting a marriage license. I had two copies done, one we submitted to the City Hall in QC and the other I sent to my husband in the US. He has it on file and will attach at least a photocopy of it to the forms.

03-13-2009 - I-864 complete, still reviewing the DS230

03-19-2009 - Under NVC final review, the operator told me that i need to redo my DS230

03-23-2009 - CASE COMPLETE!!!

03-25-2009 - GOT A MAY 7 INTERVIEW!!!

03-26-2009 - received appointment letter from NVC through email

03-31-2009 - 1st day Medical

04-01-2009 - 2nd day Medical. PASSED!

04-02-2009 - NVC sent docs to USEM

05-07-2009 - USEM Interview.. VISA APPROVED!!!

05-12-2009 - Finally got my VISA!!!

05-27-2009 - Flight to OK

Hi all!!! I'm also gathering all requirements for my DS-230. What i have are these:

1. NSO birth certificate

2. NSO marriage certificate

3. Photocopy of biographic page of passport

4. Police & NBI clearance in married name

5. 2pcs 2x2 visa photos (white background with case #, my name & birthdate at the back)

6. DS-230 form

In #20, the form didnt say we have to send police clearance of all the places we had lived since 16 y/o, right?? I am from Bacolod originally & just transferred to Makati for college & lived here since so this #20 + police clearance is making me nervous,hehe! I really hope i don't have to go to Bacolod!!

Well anyway, i have sealed my envelope & will send the package to my hubby now. Once he receives the IV receipt (& barcode), he can mail the package to NVC asap.

July 29, 2008 - Mailed divorce papers

August 25, 2008 - Received NOA2 - APPROVED!!!

Aug 27 Arrived at NVC

Sept 9 emailed DS3032

Sept 12 received IIN & AOS bill, sent check

Sept 26 received IV bill

Sept 29 received AOS receipt, sent I-864 package and check for IV

Oct. 10 recived receipt, mailed DS-230

Oct. 17 NVC received DS-230, waiting for approval

Oct. 29 approved!!!!

Oct. 30 letter sent to USEM Manila

Nov. 12 received appointment letter

Nov. 26 medical

Dec. 17 interview
