Hi All,

After hearing a lot about Rfe's on this issue i called Dr. Seiden Office and they can issue a copy for $50

so can i submit that along with my AOS?? Just to be on safe side

also on my medicals they have used my Drivers Licence As my ID is that OK??

Any n all suggestions are welcome

$50 for a copy?!!! :o Did you already have one done, and you just need a copy, or is this to transpose the information from your DS-3025 to the I-693A?

I don't think you can provide this copy with your AOS. You will need to take it to a USCIS designated civil surgeon who will transfer the info onto the I-693A in a sealed envelope which you can submit with the AOS.

The vacination information information from your K1 medical is on form DS-3025. For purposes of AOS you need to submit form I-693A (which is the vaccination supplement). You do not need to redo the entire medical if the last one was done within a year of your AOS application.

A civil surgeon will use the information on the DS-3025 to fill in the I-693A and will charge you for doing this. Yes its a bit of a rip off but what can you do?

People in the past have submitted form DS-3025 instead of the I-693A. Some have had RFE's some have not. It's a gamble, is it worth taking? Thats your call

I had my AOS interview yesterday. I sent in my vaccination worksheet from Dr. Seiden's office with my AOS package. I never got the Vaccination Supplement done and yesterday the officer said I needed to get it. I did and it all worked out the same day but yes I would get it if you can. Try to call around to find the best price, I got mine for $10.

I agree but they use the one from the K1 so thts wht i was thinking if it is the same sealed envelope..and if my sealed envelope is not there so they might issue an RFE

The sealed envelope with the visa which the consulate issued wht the hell is there in tht packet my medicals Rt??

Since a K1 had to have a medical and vaccinations before the K1 visa was issued, and since immigration confiscates and retains the sealed envelope from the fiance at the POE, why in the sam-hell does a K1 have to prove vaccinations again when the freaking USCIS already kept the info. when the fiance entered the POE?

They must be highly organized geniuses. Glad to know our tax dollars are spent so efficiently...
