hi all, i have quick question bout the process of aos application, i already have all the requirements except for vaccination supplement, i understand that when filing aos vaccination supplement is needed, i had my vaccination report from st luke's though, but am not sure if i still gonna go to civil surgeon, coz ive heard that some people still get some vaccination here..in US but with st luke's vaccination report it wasnt stated there that i need one here..i dont want to get rfe coz it would be a hassle and a waste of time too. i need some inputs..thanks

2008-07-31 : thanks God..visa at hand...yeheeeyyy

2008-08-08 : flight to US...POE SFO

10-03-2008 : send applic

10-07-2008 : recieved notice of action

01-13-2009 : biometrics appointment-done

01-14-2009 : touched

Hi . What I did is I submitted a copy of the DS-3025 that was given to us from st. Lukes and aside from that I go to a Civil Surgeon for him to transcribe the vaccines acquired to I-693. Good luck sayo

Some peeps get away with not filing the I-693 and some don't. If they RFE you for not having such it will delay you getting your GC.

The medical result from St. Luke's is valid for one year, I think. As early as you can, you should go to a Civil Surgeon to request them to transcribe the results in their Forms. There's a fee, of course. Right now, look for Civil Surgeons in your place.

I transcribe mine Bel for $65 here in Delaware. If you can see down the bottom of the DS form says check box vaccines incomplete. The civil surgeon requires me to have a documentation for the varicella coz the people in St. Lukes just put on VH there which means I had my chicken pox before but since the CS wants documentation he advice me to get a blood test or get the vaccine again. What I did is went to the City health office and got the vaccine and its for free. All in all I just had $65 expenses for the transcribe sealed envelop.I suggest you go look for a CS to transcribe it if you are still incomplete. Check the down bottom box.

..but i have another concern too ive read the application procedures that form i-693 medical examination sheet is not required if you are applying based on continuous residence since before 1972, or if you have had a medical exam based on a fiance visa..so we are planning not to file i-693 and that we'll just submit vaccination supplement from st lukes instead..is it alright?

Its up to you. Some people didnt receive RFE even if they just submitted the vaccination supplement but some received RFE for not submitting the I-693. Its upt to you to take the risk
