We are about to send off our AOS for my wifes K1. Do we really need a civil surgeon to fill out the part of the I-693 for the vaccination part even if she has a copy of her DS-3025 vaccination report from her physical done in the Philippines for her K1???

There is a bit of confusion since the I-693 was revised in May of 2008. I am not sure the definitive answer exists yet. Many have successfully adjusted utilizing only the DS-3025 prior to May. Your call.

Yeah, things are a bit confusing since the change but I've know of at least two people who had their medicals in Colombia for their K1 and K3 visas and only sent a copy of the vaccination document they received from the doctor. They recently received their green cards without an RFE.

So, if your wife doesn't need any of the following and she had her medical back home less than a year ago, she should be ok with just sending a copy of the vaccines. These are the new vaccines:

Rotavirus (children only)

Hepatitis A (not needed unless you live in a place where it's easy to get Hepatitis A)

Meningococcal (children2-18 years of age, people who have been exposed to meningitis)

Human papillomavirus (Women only, who are 26 years-old or younger)

Zoster (people who are over 60years-old)

CR-1

02/05/07 - I-130 sent to NSC

05/03/07 - NOA2

05/10/07 - NVC receives petition, case # assigned

08/08/07 - Case Complete

09/27/07 - Interview, visa granted

10/02/07 - POE

11/16/07 - Received green card and Welcome to America letter in the mail

Removing Conditions

07/06/09 - I-751 sent to CSC

08/14/09 - Biometrics

09/27/09 - Approved

10/01/09 - Received 10 year green card

U.S. Citizenship

03/30/11 - N-400 sent via Priority Mail w/ delivery confirmation

05/12/11 - Biometrics

07/20/11 - Interview - passed

07/20/11 - Oath ceremony - same day as interview

You need a CS to transcribe the vaccines taken to the I-693 form. She don't need to complete the whole 3 doses of HPV vaccine. 1 of 3 doses is enough and the CS will still sign the I-693.

You need a CS to transcribe the vaccines taken to the I-693 form. She don't need to complete the whole 3 doses of HPV vaccine. 1 of 3 doses is enough and the CS will still sign the I-693.

There's nothing confusing about the I-693 instructions on page 4, section III.

The confusion is that the USCIS may still in fact still be adjudicating based on the DS-3025
