Good question. That's the form i have filled out. I am anxious to see now.

I have not used it. I filled out the other form and it is fairly easy to complete. All the direction come with it on line. I have mine just sitting and waiting until the day comes that I can send it.

Good question! I would also like to know if it's ok to use this form if you are eligible.

I used it and it worked great....as long as you make plenty of money and you have transcripts or returns to show for the last few years its fine to use.

If you look at both side by side there isn't a gigantic difference between the regular I864 and the EZ.
