My wife came here on a K1 visa which required a medical exam and vaccinations. The papers for her exam and vaccinations were in the sealed package given to her by the consulate, and the package was taken from her by immigration at JFK. Does anyone know if it is necessary to get the vaccinations again for the AOS since we don't have the I-693 that was taken at JFK?

"If you are a K-1 fiancé(e) or K-2 dependent who had a medical examination within the past year as required for the nonimmigrant fiancé(e) visa, you only need to submit a vaccination supplement (I-693A), not the entire medical report. You may include the vaccination supplement with your adjustment of status application."

You should have form DS-3025 if your fiance took her shots overseas. What you need to do is have a Civil Surgeon transcribe that information from the DS-3025 to the I-693A. So long as she is here on a K1 and her medical was less than a year ago, the vaccination supplement is all that is needed.

We also did not have the worksheet DS-3025 from the medical. We chose for my husband to get revaccinated at the civil surgeon's, to avoid the hassles of trying to get copies of his immunization records.

You have three options, I believe:

1. Do as we did, and just get revaccinated at the civil surgeon.

2. Try to get a copy of the vaccination worksheet from your wife's medical in Paraguay, so that the civil surgeon can copy the relevant information.

3. Find a civil surgeon that will accept other immunization records (if you have them) in order to fill out the I-693A.

I would say that the above infomation is correct but also when phoning around for a surgeon make sure they are not ripping you off and telling you you need something when you do not. Good Luck.
