I-134 questions

I have a few questions I hope might be answered here.

1:On another website I was told to have the I-134 on 1 page front and back instead of having page 1 and 2 on 2 pieces of paper?

2:On that website it also said to include your bank account info. including several months of bank statements and up to a year of transactions to show regular deposits over time, I earn 40k from my job, will I still need to give my bank statements as well?

3:Should I send a transcript of my taxes of the last 3 years or can I just sent my entire 1040 tax forms (from turbotax which is easier)?

Best to have tax transcript as they show you definitely filed. 1040s don't! Only need the most recent year but doesn't hurt to have 3 years.

However the I-134 prints out from the USCIS website submit it that way.
