K1 PACKET 3

We got our NOA 1 and NOA 2 telling us the first package we sent in was approved.

I am Canadian, my fiancee is American and I'm trying to move to America.

Now, I got this "packet 3" and what I'm wondering is on the DS230 PART 1 it asks me to state ALL the times I have entered the US. The problem with that is that I've been coming to the US since I was a little girl and I'm now 20. I've been here more times than I can count. As I type this, I am visiting my fiancee in Indiana. How do I go about answering that question. If you are British or Canadian you do not need a visa to enter or visit the US for up to 3 months. After 3 months you need a visitors visa. Therefore, I've never needed a visa and have no way of telling how many times I've been here let alone the exact dates of entry and return. I've always come over legally and been legit with passport and all but as a little girl you didn't need a passport, and half the time when I return to Canada they do not scan it.

Also, I have 2 middle names. On my passport it has only my first middle name, whereas on my birth certificate, it has both. Do I put my second middle name under "other names?" or put them both under middle name?

Please please help.
