Hi

Im completing the I-485 forms and i see that it suggests to send in a vaccination supplement if you are a K-1 visa holder and had a medical done for this.

I had my medical done for my K-1 Visa which i remember included a vaccination supplement amongst the forms and then all these forms were sent to the American Consulate.

My question is that since the forms were sent to the consulate i do not currently have a vaccination supplement, so is it necessary for me to obtain one again?

Im afraid to say that yes, you do need to get the vaccination supplement (I-693A). It basically copies the information from the form at the embassy on to a new form for which the Doctor will charge you for. It has to be done by a Civil Surgeon. You can find one in your area here:

Shop around for who will give you the best price. Do you have a copy of the vaccination form done at your medical in New Zealand? Thats the best thing for them to take to copy. If you dont have it you could try getting the doctor that did the medical to fax you a copy if they have it on fike

What was included in the paperwork You turned in at the POE was a vaccination work shett. Not the supplement. You should have been given a copy though. You are required while doing the AOS to go to a civil surgeon designated by the USCIS to get the supplemt vaccination sheet filled out. As long as it has been less than a year since Your over seas physical You are not required to take that again. The civil surgeon uses the worksheet to fill out the supplment.

Has anyone else tried this instead of paying a Civil Surgeon to transcribe the information from the DS-3025 to the I-693A? :unsure:

We "tried" (unintentionally) doing that (sending DS3025 only) because of confusion between the vaccination supplement and worksheet and got an RFE a couple of weeks ago. The worst part is that the RFE asks for I693 instead of I693A. It must be a mistake because it's only been a few months since her overseas exam.

This whole thing really confuses me!!!! I am having trouble with it myself! My fiance said the only thing the doctor gave him was a sheet saying which vaccination he needed to get. then when he got the vaccine he got another form saying he received it! Is this what he needs or does he need to contact the doctor before he leaves and have them send him something else this 693 form or whatever it is! I am just glad that I am not the only who had this question! please if you can help i would appreciate it! he is trying to get everything he needs before he comes next Saturday!!

Oh I am sorry I forgot something. My fiance said that this sheet he has is in German!! Isn't it supposed to be in English. A civil surgeon won't be able to translate it so this can't be the correct sheet right??? I am so confused :bonk:

We tried and I was willing to take the couple months delay if an RFE was issued - none was issued and we got through OK. My advice is that if you are able to take a couple months delay it might cause, then and only then try it. Good Luck

Thanks to all who responded. Yes, it seems like the way to go is just get the supplement filled in. Im going to check with the medical center who did my immigration medical and see if they have that vaccination sheet for the civil surgeon to copy. May also try giving USCIS a call to clarify (let you all know if i get an actual answer on this). Cheers.

I was thinking about you when I posted. I believe you also said you know of people who got an RFE, like someone else on this thread, yet another person didn't. It definitely sounds like a 50/50 chance that either they will accept it, or they send and RFE and delay processing. I think I'd rather not take the delay. If there were more people who said they had no problem, I might chance it. One poster said it only cost $10 to have them transcribe, while I think I read someone else saying $25. The lack of consistency is very annoying! Really makes you wonder how this government gets anything accomplished! :no:
