I've just read that I should have been given that I 693A form by the physician who conducted my medical examination for the visa. He gave me nothing. I read on this website that sometimes that form is in the sealed envellope I gave to the Consulate at my interview. If that's the case the Consulate didn't give me anything back. They only gave me an envellope to give give to the Immigration Officer at the POE. Which I did.

It seems that I need this I 693 A form for AOS. Since I don't have it, and didn't even know until today that such a form exists, how can I do for AOS?

Can you give me any hint?

May 21st 2006: Entered the USA on a K1 visa

July 2nd 2006: Marriage ceremony

Oct 10th 2006:Received Green Card

REMOVING CONDITIONS

August 12th 2008: Sent I 751 to CSC

Dec 5th 2008: Green Card in the mail

NATURALIZATION

Dec 4th: Sent N 400

March 18th: Interview

May 24th 2011: Oath in Minneapolis. Journey is over.

What you read was either wrong or you mis-understood it. I-693A is a USCIS form used in the USA, the comparable form that you should have been given at your visa medical exam is DS-3025 Vaccination Documentation Worksheet.

You can visit a USCIS-approved Civil Surgeon in the USA to get an I-693A. Take your vaccination records with you.
