Hey everyone,

first of all this site is amazing, thank you all for contributing to its wealth of knowledge. :)

I had a question regarding the I-693 "required" for the AOS application. If I had a medical in Canada for my K-1 in April do I need to submit the I-693 form? I am a bit confused by the instructions. maybe I'm not reading them right.

I have a vaccination record that they gave me in Canada, what would happen if I just send that? or do I have to visit a civil surgeon no matter what? and if I do just send that in then will I get an RFE? or will it be rejected?

AOS application based fromk-1 visa still needed to submit I-693 PART 1, 2 AND 5 which will be completed by the Civil Surgeon here in US. You should alsosubmit the vaccination record from Canada. I've read some cases in here wherein they just submitted the vaccination record from countryof origin and some got lucky and dont have an rfe for I-693 and some does. So its up to you to take the risk I guess. Its better to be more than complete than sorry right. Good luck and blessings to you

Sorry its Part 2 no. 5 vaccination. So all in all its Part 1 (information about you usually filled out by the CS too), Part 2 no. 5 vaccination part (your vaccination record obtained from canada will be transcribed in here ) and Part 5 for the CS signature.

Okay thank you for your response! I will try and call up a civ. surgeon tomorrow and see the cost. Thanks for your help!

DOn't be fooled by the propositions of some CS which will tell you that you will need to do the medical examination again, etc. TRy to call as many CS to look for a lower price im assuming just for the any other vaccination that may be required for you. GOod luck :thumbs:

Hey everyone,

I am ready to mail the AOS package but I'm didnt do this part because I read in the sticky and the guides that I wouldnt need to. I do however, have the document given to me in Canada, and I thought that would be enough. But I re-read the instructions and they make it sound like I should submit the I-693 document. so I am wondering if I should go ahead and make the extra effort to find and contact a civil surgeon.

7/14/08 SSA called me in regards to problems at local SSA office

7/15/08 INFOPASS appt to fix POE name input error and then went to airport to fix POE name error

7/17/08 Received SSN card in mail and went to get state ID

7/21/08 State ID received in the mail and tested again for measles immunity

7/29/08 MMR shot #2 given and dropped off papers at Civil Surgeon (CS)

7/31/08 Picked up I-693, completed by local CS, (I paid $25, don't be fooled)

8/3 - 8/6/08 Worked on forms I-485, G-325A, I-864, I-765 and I-131; sent to Chicago - YESSS!

Hmmm...Pushbrk you almost always say the right thing but I have to disagree with you in here..Like myself, I submitted AOS based from K-1 and the I-693A is not being used anymore. It specifally said in the instructions page 4 Section III on Frequently asked questions http://www.uscis.gov/files/form/I-693instr.pdf

"If you were admitted as K1,k2,k3,k4 you will not required to have another medical examination. You will however, be required tocomplete PArt 1, Information about you, and submit the vaccination section of Part 2 with your adjustment of status application. A designated Civil Surgeon must complete the vaccination section and PArt 5, the Civil Surgeon's Certification."

Perfect. Then you go to a civil surgeon and have him do his thing. He'll do it on the proper form but make sure it's only the vaccinations and not a full medical report. They like to charge for those.

The safe way is to file it with the I-485. We didn't on purpose because we filed earlier than we really wanted, just to avoid the hefty fee increase but wanted to make sure any interview occured no sooner than 4 months later. As it turned out, there was no RFE and the interview was almost six months later anyway.
