What the H*** is an 'IV-15'?????

Timeline

After waiting for an eternity (or so it seems, actually about 15 months since we started the process) I emailed the Embassy to find out when the visa interview was going to be granted.

I got an email back today with this info:"The Biographic Data form DS-230-I, the DS-156 and supplement DS-157 and the IV-15 document checklist were sent to your fiance on July 13 2005. We have receive all of the required forms except for the IV-15. No further action will be taken until we are in receipt of this form."

I have looked and looked and do not find any reference to an 'IV-15'. I am going crazy with trying to find it and can't take it anymore... PLEASE PLEASE, can someone help me on this!

SO, they sent the packet almost a year ago? did you see any of the packet and send it back? Are they saying you're just missing just one form? Have you done the medical, et? More info. please ;)

10/07/2006 WEDDING DAY!

11/14/2006 AOS packet made it to 'the box' after being overnighted.

12/02/2006 Paul had biometrics

12/14/2006 AOS Forwarded to CSC AND AP Application approved.

01/17/2007 First touch of 2007 at CSC

01/20/2007 Touched AGAIN (also the 18th) come on...

February: Oops, RFE for a REGISTERED marriage certificate. Oops! Overnighted it.

02/28/2007 Paul gets email letting us know his GREENCARD is on it's way! It's done...for now!

03/09/2007 Paul's greencard arrives. And breathe...

We began with mailing the I-129 in on February 27, 2006 so the whole process took us approx. one year.

We have sent all the paperwork, Billy has had the medical and it is on file at the embassy, we waited almost 12 weeks (the first time) for the visa interview only to hear back that they had NEVER RECEIVED the paperwork. We sent the paperwork again about 4 weeks ago. I got impatient and emailed to ask about the interview, and received this email back saying everything is there EXCEPT the IV-15. I have no idea what that is and haven't seen anything (after searching visa journey, embassy site, etc.) referring to an IV-15 anywhere. But without it... nothing will procede!

2006-04-28 – Self collected Packet 4 at Embassy and make appointment for medical

2006-04-29 - Medical check-up

2006-05-11 - Interview Day!!!! - APPROVED!!!

2006-05-15 - Submitted my resignation from work

2006-05-17 - Got a call from Embassy, my visa issuance ON HOLD due to new law IMBRA, they are not able to issue my VISA!!!

2006-05-19 - Pick up my passport without VISA. No information as to when my VISA will be ready...

2006-05-22 - Embassy inform that our case has sent back to NVC via FedEx

2006-06-24 - Last day of my job after serving 1 month notice. (Manage to extend my last work day to 6/24)

~~~ Still don't know what they need from us...

2006-06-25 - USC's finace fly to Malaysia...

IV-15 is just exactly what the e-mail you received says it is. You can find it in the US consulate in London's web site if it was missing from your visa application package. (Hint - you want IV-15V)

It sounds like you are missing the checklist that your embasy sent with the third pakcket. My fiance did not go through London but on his checklist it said that an interview date would not be assigned until it was signed and sent to the embassy. It will have a list of all the things you need to bring to the embassy for the interview....birth certificate, passport, affadavit of support, evidence, police certificate, etc.. If this was not signed checked off and sent back you would not receive an interview. I hope that this somewhat helped! Good luck to ya. I f you have anymore questions just let me know!

Timeline

As already mentioned, the IV-15 form is the document checklist. It's also the formal request for an interview. When the foreign fiance(e) signs it and returns it to the consulate, they are in effect confirming that they have all the requested papers and are ready for interview.

The consulate (at least in London) will not schedule an interview without it.

If thats what they are missing, just go to the London Embassy website & download another, then send it to them!!! :yes:

26-June-2006- Social Security# arrived in the mail....YAY!!

Hi Halfbohemian .. I've sent you my copies of the IV-15 via email. There are 3 pages - first two are a checklist of sorts that you have to tick off each thing that you have documentation for. The third page is where you sign off on all that. This form would have been part of the cover sheets that came with your fiance's packet. Check your email and you'll see my samples of the form. You can do what the others said and get blank copies of this document since mine are only images for you to look at .. by going to consulate's website .. good luck hun!!

I've taken some time away from this board and to heal from the last 3 years of relationship and hard work as we brought him here - and am going to post my experience so people who have experienced this can find some support. Thank you for all the replies! I do appreciate all the help. I guess the checklist that was requested was sent with the first mailed packet that was 'lost' in the mail. When the Embassy told us that the forms had not been received, they told us to send:

'you may wish to resend the forms DS-230-1, DS-156, DS-156k and DS-157 to the Immigrant Visa Unit as they appear to have not been received.

Your medical examination results are on file in the Immigrant Visa Unit.'

So we sent the forms requested.

THEN after 5 weeks, I got impatient and emailed to find out when the interview would be scheduled. Then and ONLY THEN were we told this 'IV-15' form would be needed before the visa interview would be sent!

Billy emailed a request for another 'checklist' since I can't find heads nor tails of anything that says 'IV-15' anywhere. (Am I just not looking right? or what?)

Anyhow, thanks again for the replies and help...

I believe that that form is a checklist - with tick boxes -and that should have been sent back. Hope this helps
