I-134 Form, Previous submission?

I'm filling out the I-134 form for my fiance. Question 9 asked if I have previously submitted the I-134 form. I did for my fiance when we tried to get him a visitor's visa. Should I put his name down? Or just leave question 9 blank -- repeating his name over and over again on the same form seems silly.

I would think if the visa was refused so was the I-134 so therefore it does not count...

It's not going to count against him in the household size calculation but the question asks about submissions, so you answer it accurately. A lot of this process seems silly but it's far more silly to fail to answer such a simple question accurately.
