Hi,

I'm a little confused about form I-693. Does that form need to be submitted with the I-485 AOS? Reading in the instructions it says "If you are a K-1 finance who had a medical examination within the past year as required for the nonimmigrant fiance visa, you only need to submit a vaccination supplement, not the entire medical report."

Form I-693 is from a doctor here that has performed a medical exam, correct?

Carlos received a card that has the date he received two vaccinations. Is that the vaccination supplement that's requested?

This process is just as confusing as the application for the K-1! Thanks in advance for any guidance and help.

You may not need a I-693 but you will need to go get a I-693A which is the supplement they are talking about. Take the vaccination sheet your civil surgeon in your home country gave you and bring that to a civil surgeon here (go to the USCIS website to locate surgeons in your area) who will transfer it onto a I-693A. If your shots arent up to date, you will be required to get them. The fee can vary between 25 to 50 dollars for the supplement (not counting shots), depending on who you go to.

Are all adjustment of status applicants required to have a medical examination?

Most applicants for adjustment of status are required to have a medical examination. The medical examination must be conducted by a civil surgeon who has been designated by the USCIS. The examination must be documented using Form I-693, Medical Examination of Aliens Seeking Adjustment of Status.

EXCEPTIONS:

2. Ks: If you were admitted to the United States as a fiance(e) (K-1), child of a fiance(e) (K-2), Spouse of a U.S. citizen (K-3), or child of K-3 (K-4), and received a medical examination prior to admission, then you do not require another medical examination as long as your application for adjustment of status (Form I-485) is filed within one year of your overseas medical examination. You will, however, be required to submit a vaccination supplement with our adjustment of status application. The vaccination supplement must be completed by a designated civil surgeon.
