Hey guys!! We just had our K1 interview approved yesterday and I want to make sure my fiance brings everything we are gonna need for the next steps with him. My real only question regarding this is the form from the medical that the application says that he needs. It says he needs to send a copy of a vaccine form that he got from his medical in Frankfurt. The thing is he said that he never received this form from the doctor! He has his vaccine pass from Germany but will that be good enough???does he have to have the doctor fax this form??Also do they need a copy of my(USC) passport?? I don't have a passport, would that be a problem?? Thanks and I am sure I will have more questions to come! Will this paperwork ever END!!! :no::wacko:

The USC not having a passport is ok. In this case I would assume Your fiance traveled to meet You. A passport is just another means to prove Your citizenship besides Your birth certificate. Your fiance should have been given a copy of the Vaccination Worksheet. If not just make sure all the shot records are brought. As Your fiance will have to go to a Civil surgeon once in the states and get a vaccination supplement filled out that will be turned in with the AOS packet. Try to get a copy of the worksheet before he leaves Germany.

Thanks! yeah we met while he was here ona J1 so thats why I never had to go see him. He did mention that he got a paper with the vaccinations on them but it was in German. Does he have to translate this then or does he just go see a civil surgeon and they take care of it for him??

My husband is from Austria, which of course means his immunizaitions were also in German. They will need to be translated into English on an official immigration doctor's form. We had a copy from his K-1 interview which the whole file is lost or in transit from Missouri, the INS officer did not accept the copy he had translated onto the Pima County Health Depart immunization card, she said it must be confirmed and trasnlated by an Immigration doctor.

YOur passport is a good thing to take, she asked me if I had traveled to Canada to visit Anton and lucky I had one stamp in my passport to prove that I had been there, cause word of month just doesn't cut it.

There are immigration doctors in every city. Actually here in Tucson it's basically medical doctors I guess that work with INS.

Designated Civil Surgeons

Most applicants for adjustment of status are required to have a medical examination. The medical examination must be conducted by a civil surgeon who has been designated by the Bureau of Citizenship and Immigration Services. He did mention that he got a paper with the vaccinations on them but it was in German. Does he have to translate this then or does he just go see a civil surgeon and they take care of it for him??

Have him look on the paper he received and ask him whether it says DS-3025 on it.

If yes, then that's the vaccination worksheet, and with it you can go to an USCIS doctor in the US and have the vaccination supplement filled out.

The doctors in Frankfurt don't know (or don't care) that they are supposed to give every person a copy of the vacc. worksheet (it says so in bold print on the bottom of the form: "Give copy to applicant" :whistle: )

I did get my copy of the DS 3025 but only after asking for it and insisting on it. I know several people who did not get it initially and had difficulty obtaining it afterwards (specially from the docs in Berlin).

But even if he doesn't have the vaccination worksheet, it is usually still possible to have the vaccination supplement filled out in the US if he has all the paperwork that shows he has been vaccinated (or alternatively, gets vaccinated again :( )
