My SO has a copy of her official vaccination record is this good enough to get a vaccination supplement filled out by a Civil Surgeon here?

I cannot speak for other people, but we used my wife's RUSSIAN records at the civil surgeon we went to. It helped that he read and understood Russian. I cannot say if this was within his legal rights, or if you can do it in your area.

02/09/06 - NOA1 date

12/17/06 - Married!

AOS Process a fading memory...

01/31/07 - Mailed AOS/EAD package for Olga and Anya

06/01/07 - Green card arrived in mail

Removing Conditions

03/02/09 - Mailed I-751 package (CSC)

03/06/09 - Check cashed

03/10/09 - Recieved Olga's NOA1

03/28/09 - Olga did biometrics

05/11/09 - Anya recieved NOA1 (took a call to USCIS to take care of it, oddly, they were helpful)

I did have official one from MOM and guess what when i arrived here and went to civil surgeon in San Francisco they told me i need bunch more,since MOM was out of it..(Bastards)so MR>civil surgeon quoted me couple vaccinations for 350usd!!!I said no and went to Kaiser,since i had insurance there and got it for 10baks...then had to go back to civil surgeon though...so yeah you can go with that form to Civil surgeon,but you can not be guaranteed treatment you hoped for...(sucks)

The CS here seemed like a lot of it was left up to his discretion. He "trusted" the word of the dr. in Russia but wouldn't trust the word of my wife who claimed she'd had the (pointing to her arm in several spots) "cheeek, cheeek, cheeeek" chicken pox as a child.

He said (Indian dude) "I don't care if you hada the cheek, cheek, cheek, here and here on your arm. I need to see the word of a dr. saying you had the cheek, cheek, cheek."

So, a few more appointments and a lab test later, he signed off on the I-693A that we sent in.

Ensure your beneficiary makes and brings with them to the States a copy of the DS-3025 (vaccination form)

If the government is going to force me to exercise my "right" to health care, then they better start requiring people to exercise their Right to Bear Arms. - "Where's my public option rifle?"

She had vaccinations done at her medical, forgot to take the vaccination supplement (DS-3025) with her to have them fill it out, however, she did get a record from them in English that spells out the vaccinations received.

they will make her do it all over again-more likely..If she has anybody there who could pick it up for you and fax you would be nice..
