Check the site:

1010USD (fee covers biometrics, I-485, I-765, I-131)

355USD (Form I-130)

Most doctors will accept overseas vaccination records, but you must show them physical proof. If you can't show it you may get all shots needed (after August 1st vaccination requirements changed so check the USCIS site)

Medical Examination I-693 (depends on how much the doc charges you ranges from 125-400)

Don't let the choices chose you, chose the choices that you want.

Two checks. One for $1010 that covers everything but the I-130 and another for the I-130.

I brought my vaccination record and I didn't need any of them. I would have gotten a shot if I had mailed the docs after the new vaccination requirement came into place.

I hate needles too, though remember you'll still need to get 2 mandatory: blood test and TB skin test. (everyone has to get those two)
