What caused the error? Do you know?

Two things come to my mind. You can file an I-90 and pay a fee or if you can live with it for two years get it corrected when you remove conditions....

I would call them or make an InfoPass to see if your name is correct in their system and the mistake was just a case of name being longer then allotted width on card. If it is actually a mistake in their system, you can File an I-90 with NO FEE, if you can show it was an administrative error on their part. You can do this by sending a copy of your previous applications that show you spelled your middle name correctly on the form, backed up with copies any other identification (ie passport) that has the correct spelling.

When I received my original GC my middle name was spelled wrong on the card and I followed the above process and had a new card in about two months with name spelled correctly and did not have to pay a fee.
