Basically, Pakistan's insistence on a "hate India always" policy over a "love Pakistan" policy (advocated by several leaders from erstwhile East Pakistan particularly)--the consequences of which have been the separation away of East Pakistan as Bangladesh and current internecine conflicts in remainder Pakistan--and their refusal to learn from it.

• 1996-2001 by backing/training/"safehousing" Taliban, AQ and other militant groups
• post-2001 by turning blind eye to the same groups activity in "neglected areas" of the country
