RFE- medical and immunization records

Hi! I filed my AOS package about 1 month ago. I already recieved my biometrics appointment. However yesterday I received an RFE for my immunizatiion records. However I am ver confused, since I submited my medical examination which was seales and I figured that the immunization records would be inseide the envelope. Does anyones have any ideas? I am definetly going tomorrow to the civil surgeon and i will ask her for help.

Hi did you also submitted the DS-3025 copy (vaccination supplement) that was given to you in your medical and vaccination from your home country?

I did not have any records from my home country but I got all the immunizations here in the USA when i got the medical. Was I supposed to include a copy of the paper that has a list of the immunizations I got? I thought that was included in the I-693 form??

Oh ok if you dont receive that in your home country then you dont have it. And if you got all the immunizations here in the US then the CS should input that in the vaccination part of I-693. Didn't you have a copy of your I-693? if not you can try to contact your CS and ask what are the vaccinations that he/she gave you. Can I ask when did you had your medical and vaccinations here in the US? Because there's been updates with immunization effective july 1st

Thanks for the info Richo! I actually already got all those immunizations and i got the tb skin exam, however I did not submit a copy of the records of immunization because I thought that was inlcuded in the seales envelope. I will just submit a copy of my immunization records and hopefully that is everyhting they need! Getting RFE makes me sooo nervous.. lol!

Oh ok I got nervous with RFE'S too..hehe..Don't forget the original NOA for RFE and make copies for yourself..good luck

I mean you need to include the ORIGINAL NOTICE OF THE RFE and retain a copy for yourself. I dont think you have to include the other NOA1 but it would not hurt to send copies and not original of those..BUt dont forget to submit the original notice of the RFE.

when you scroll down, you'll see Request for Evidence NO.4 states

4. Always return the entire original request for evidence (color copy) with the complete response to the request for evidence. DO NOT mail portions of the original request for evidence separately. Make a photocopy of the original request for evidence for your files. Returning the original RFE will make the response easier to identify in the incoming mail and get the response filed with the file as quickly as possible so that the processing of the case can continue

HI! Ok so I had to go back to the doctor to get my second HPV and a few other vaccines that needed reinforncment. I send a copy of my vaccination record with the RFE yellow paper and all that. Now I see that there is a Form DS 3025? Am I supposed to fill out this form? Or will a copy of my vaccination records be enough? I got all my vacination here in the USA so it is all in english and no traduction is necesary... Please help!!!

And by the way I had to pay $200 more! I have already spend over $800 get this medical exam and get all the vaccines and the TB skin exam! It is outrageous!! And I can not believe that they are making us take the cervical cancer vaccine, first it is very expensive.. but not only that it is a new vaccine and there are not a lot of tests on this vaccine. Cervical cancer is not contagious and it is stupid that they are making us take this!! Even the nurse who helped me today said she thought it was completely outrageous..she told me this vaccine is so new that she is not willing to have her child take it... it all comes back to the $....

Hi again! Good thing its all done. I had the first shot of HPV vaccine too and don't intend to finished the next two does I think its best that you have the CS fill out the I-693 form and for him/her to input the vaccines in the form and have him signed it and put it again in a sealed envelope. Just a suggestion Or maybe that's what the CS already did? :whistle:Kinda like doing the I-693 all over again but this time with the vaccines included.The DS-3025 (vaccination worksheet) was given to me when I had my medical in my homecountry and since I didnt have it in there the DS-3025 is blank but has the physician's signature on it , but I still included it in my AOS application anyways. SInce you said that you don't have that before and you had the vaccines done here in the US then the I-693 form is what you need to be signed and completed by the CS.

hey richo!! thanks!! Hey but wont they check your immunization records at the interview to make sure that all the vaccines are up to date?? I am just wondering, because otherwise I do not want to get my last dose for the HPV.
