I call NVC this morning and the AVR says that my financial support documents have been reviewed and have sent out a checklist for additional information. I call NVC and speak to an Operator, I am told that I did not write my name in question #1 of the I864 form. Very sad....how could I not enter my name in question 1. I checked to copy of what I sent and my name is missing. I'm pretty sure no one has ever made this mistake before.

Questions:

Should I just use the Bar coded Coversheet from my AOS Fee invoice? The operator said it's ok to use.

Should it be Attn: to CMR or DR? The Operator said that I could use the same address as I did the first time to mail out the I864 package

Do I need to send another cover letter with a checklist? This time I will only be sending the I864 form as they have the supporting documents.

Any input is appreciated....
