Hi!

I am about to send my application for AOS + EAD + AP and I have the following question about the Vaccination Supplement sheet that is required.

When I had my medical examination for my K1 visa they gave me a sheet called "Vaccination Documentation Worksheet"(DS-3025 apparently) (it says on this sheet "for use with DS-2053").

Is it the sheet that I need to submit along with my AOS application?

What about the medical documents that are in that big brown enveloppe that I still have and that was never opened?

Hi there good luck in advance. Yes include that DS-3025 vaccination supplement together with the form I-693 signed and sealed in an envelope by a CIvil Surgeon with your AOS application. The brown envelope "must not be opened". You will need to handed it to the immigration officer upon your entry here in the US and they will get it all.

I don't understand....I read at several places that the results of the medical examination that I had for my K1 visa were good for a year (I had that examination a couple months ago) so that I didn't need to have another examination when applying for AOS....

Can someone confirm?

Some submitted the DS-3025 w/their AOS package and some submitted the DS-3025 with I-693. We chose to submit both DS-3025 and the I-693 to avoid an RFE (fingers crossed). My husband (then-fiance) came on a K-1 and he had his medical in May 2008 but I knew he needed the 2nd series of the Varicella and also the TD shot, so we went to a civil surgeon (found on the USCIS website) and got his shots there and also got the DS-3025 transcribed for $20 (shots additional, of course).

Weren't you asked for the big brown envelope that says "do not open" at your POE? I'm surprised you still have it...unless it's just the xray film??

Also there is a new vaccination requirement which may force you to have additional shots. This is something that changed at the beggining of August.

When I filed for my AOS, our lawyer didn't include the separated vaccination paper, she said that we'll wait if the USCIS ask for it. I didn't have an Interview nor asked for RFE or vaccination.

I guess you can submit the photo copy of that, so incase they need again, you have the original..

Yup, check the NEW requirements which all that apply now require to have those new vaccines done.

Hi. Here are the directions for the new I-693. Technically one is supposed to send in the I-693 with the vaccine section transcribed from the DS-3025. The directions are linked below. Some people are choosing to send in just their DS-3025 and others are sending in the I-693 per the directions. Decide for yourself. Some people are getting RFEs for not sending in the I-693 and others are not.

7/2/08 Received David's health insurance cards! Yay!

7/3/08 Went back to SSA; they said come back 7/9 if he still hasn't received it ----- TRIP 3

7/8/08 Got list of vaccines he still needed from CS by fax/phone; tested for immunity to MMR (only vaccine still needed) at our family dr

7/9 & 7/10/08 Went back to SSA, card still not received - Denied opportunity to apply for replacement ----- TRIP 4; Went back to SSA again to apply for a replacement card - Talked to a supervisor this time, POE people entered his last name incorrectly ----- TRIP 5; went to get David's ID thru the State, denied - instructions wrong on website; measles test inconclusive, another test needed; requested new lease

7/14/08 SSA called me in regards to problems at local SSA office

7/15/08 INFOPASS appt to fix POE name input error and then went to airport to fix POE name error

7/17/08 Received SSN card in mail and went to get state ID

7/21/08 State ID received in the mail and tested again for measles immunity

7/29/08 MMR shot #2 given and dropped off papers at Civil Surgeon (CS)

7/31/08 Picked up I-693, completed by local CS, (I paid $25, don't be fooled)

8/3 - 8/6/08 Worked on forms I-485, G-325A, I-864, I-765 and I-131; sent to Chicago - YESSS!

8/11/08 NOA1 for AOS, EAD and AP; 9/2/08 Transfered to CSC; 9/4/08 Biometrics

10/24/08 Rec'd AP; 10/27/08 Rec'd EAD; 11/24/08 Rec'd GC

Your k-1 medical was before June 1, 2008, so you do NOT need to receive the new vaccines.

Some people only sent DS-3025 and get approved, others send the I-693 signed by a CS. You don't need the full exam again, read the instructions for the I-693... Section III, line 2.
