Hello all:

For the question other names used, the beneficiary has only one official name (first name only - no last name for some erason). However, within the confines of her company and in some initial bank records, she had used a first name and last name as the system needed it. This was not registered in India. However, all her official docs - Birth cert, School leaving cert etc... has only the first name - which is official. Am planning to fill the official name in Q1 and in Q7, in II-129f, planning to fill out the first name and last name as other names used. Are there any concerns? and should I show any proof for the other names used such as a a registered name or should I just leave it blank? How do I address it. Thanks.

Thank you. I will just list in the other names - her first name and "unofficial" last name. I presume it will be treated like a nick name.
