Hello everyone!!! I just want to seek help because I am totally confuse about the G-325A form. Should I use my maiden name to fill out the said form or my husband's last name? coz a friend of mine told me to fill out the G-325A form as a single and not married. But there are questions asking about the connection of the G-325A either for naturalization. permanent resident or others...I check the sample form of G-325A here, it was different as what my friend told me...that was why want to make sure that I filled out the form correctly before printing or send it to USCIS office.

And, also about the documents to be attach with AOS, EAD, G-325A and Affidavit of Support. Should we send original documents or just copies?

Please help me!!!!!!! We are planning to file this month but we are confuse what to do... :crying:

06/29/2015 - Received a text from USCIS case is received

07/25/2015 - Received NOA1

11/04/2015 - I-130 approved

11/09/2015 - Received NOA2

12/16/2015 - Case sent to Dept of State

12/21/2015 - NVC received my case for visa processing

01/09/2016 - NVC Welcome Letter Received

01/09/2016 - Paid AOS fee and selected agent

01/13/2016 - Reviewed DS-261 over the phone

01/14/2016 - Recieved invoice for IV fee

01/15/2016 - Paid IV fee

01/19/2016 - Mailed financial/supporting documents to NVC

01/20/2016 - Submitted DS-260

01/22/2016 - Email notification from NVC that they received the financial/supporting documents

02/29/2016 - Email checklist from NVC

03/09/2016 - Responded to NVC checklist (mailed)

03/15/2016 - Additional document received by NVC (another 30-45 days wait...ugh!!!!)

04/12/2016 - Case complete...waiting for interview/medical schedule

04/14/2016 - Received email for Mom's interview schedule, 26th of May

04/24/2016 - Registered Online for upcoming Medical exam appointment, May 11, 2016

05/11/2016 - Medical exam. Passed

05/26/2016 - Interview. Under administrative review.

06/09/2016 - Visa Issued

06/15/2016 - Received text notification from 2go for visa delivery

06/17/2016 - Visa received on hand

06/28/2016 - PDOS seminar

07/17/2016 - Mom's flight to USA ???

Use married name and put maiden name under other names used. Why would you fill out the form as single and not married? You are married!~!~

K1 denied, K3/K4, CR-1/CR-2, AOS, ROC, Adoption, US citizenship and dual citizenship

use ur new last name ( husband's ) then use ur maiden name on the a portion "other name used".. submit the xerox copy. save your original copy for your file. some cases requires RFE or need to submit documents so you are ready to give it.

11-01-07- My fiance got a letter from NVC that within 2 weeks Manila Consulate will send my Packet

11-20-07- Date of my Packet, actually the real one was earlier but it was sent somewhere, good thing my Fiance arrived to spent Thanksgiving day in Phils and we checked the status in Embassy.

11-23-07- Medical though the exact date was Nov. 29..

12-06-07- Interview

12-16-07- Visa in hand

01-16-98- Attended CFO

01-18-08- Arrived in USA , POE-Guam

03-16-08- Wedding

AOS

04-17-08- Submitted the AOS

04-24-08- NOA

05-13-08- Biometrics

05-27-08- Transferred to California

06-14-08- EAD card received

07-21-08- Date of AOS approval but got the letter July 24

No Interview, No RFE and Additional Vaccination( just used the SLEC medical docs)

07-22-08- Date of Permanent REsident CARd , got it July 29

07-31-08- apply for SSN

08-05-08-SSN delivery

01-12-09- First day of work(Thanks to Marielle and Mario)

The directions are all on the USCIS website. We were reading them all last night and it is very

clear about what to do.

As far as I've heard you need to submit copies and when they want to see the originals, they will ask you to show them during your AOS interview... but thats just what I've heard from others, don't know it its really like that :S

N400 Timeline:

12/14/11 - Sending out N400 package

12/19/11 - Received by USCIS

12/21/11 - NOA date

12/22/11 - Check cashed

12/27/11 - Received NOA

02/06/12 - Received yellow letter (pre-interview case file review)

03/13/12 - Placed in line for interview scheduling (3 yr anniversary)

03/17/12 - Received interview letter

04/17/12 - Interview - No decision, application under further review

04/17/12 - Biometrics

04/25/12 - Placed in line for oath scheduling (so I'm approved yay!)

04/27/12 - Received oath ceremony date

05/09/12 - Oath ceremony!!

IMPORTANT!

Make TWO copies of the entire package before you send it in. This includes the money orders too. You want to have a perfect replica of the package you are sending in. All Forms that you submit must be originals with original signatures. Supporting Evidence that you submit may be photocopies. Retain ALL original supporting Evidence since the USCIS has the right to check them by issuing an RFE (Request For Evidence). If you receive an RFE, follow the direction exactly, and make two copies of what you sent back. During any future interviews the USCIS may also want to examine the original supporting evidence.

affidavit of support is ALWAYS an original certified, notary, copy...

Form I-864, Affidavit of Support, necessary for the Adjustment of Status, does not need to be notarized.

Form I-134 does.

Each form (AOS, EAD, AP) needs to stand on its own, with its own set of supporting evidence, so yes, you need to provide several copies of each document if it is asked in several forms. From G-325A does not require any supporting evidence.

thank you to all who replied and answer my questions...but i still have an additional question..its about the new revise vaccination...my husband called a surgeon today for my i-693..but the lady told him that i need to have the TB, HMv (Human Papillomavirus) shot. I was confused and unsured because

I am surprised about that.

Who undergone these vaccinations?! If you did, how much was the fee you paid for it?
