u should have the original copy with u coz u need to submit a copy of it for ur AOS..goodluck!

Love is an unexpected thing...and when it arrives grab it with all ur heart....it may pass u jsut once.......

11.20.07 : mailed I130

01.23.08 : NOA1

04.21.08 : mailed I129F

04.22.08 : NOA1

05.14.08 : NOA2 both for I 130 and I 129F

06.26.08 : medical exam 2nd day PASSED!!

07.09.08 : interview day! PASSED! PINK SLIP!

07.15.08 : visa on hand!!!

08.01.08 : POE SFO (home with my papi!)

You should still have your original immunization records that you have had since childhood. In addition, at your medical did you receive a copy of the DS-3025?

From the doc I received

1) "Vaccination Documentation Medical Wroksheet Two" which shows my immunization record. Its signed and dated by the doctor.

2) the xray

I submitted a copy of #1 but i still received an RFE saying I didn't submit the proper immunization record.

I'm confused as to what they want.. I hd actually thought my medical info would be in the envelope I gave at POe..

Are the vaccinations complete or are you missing some?

Other than that, the only thing I can think of is to get worksheet #2 transcribed to the I-693 Part II, Item 5.

The doctor checked off two boex:

- vaccine history incomplete

- applicant may be eligible for blanket waiver because vaccination not medically approproiate as indicated above

Is the vaccination document that I submitted the correct one? It doesn't say DS-3025 anywhere...

The DS-3025 has the indicator in the lower left corner and is titled "VACCINATION DOCUMENTATION WORKSHEET" For use with the DS-2053.

I assume that since there is no notation the adjudicator has no idea what you have given them. You should probably get a US civil surgeon to put on the I-693. google Dr. L. E. Arnold. Her office has been known to do it via mail for about $20.

I looked up the form on line and mine is VERY similar but not 100%. Is there any canadian out there? I want to see if my form looks the same as yours...here are some details:

top left hand corner "For Use in Canada only"

main title: "Vaccination Documentation Medical Worksheet Two"

bottom left hand corner "Medical Worksheet Two"

Bottom center: Give copy to applicant

main body includes name, exam date, birth date, passport number, alien (case) number, immunization record, etc
