Hello everyone,

I am helping my friend who is under K3. She will have her medical this month but did not receive her packet yet. What DS form she will need to fill out and how many? Can she go to St. Luke for her medical without an oppointment letter? I would appreciate any inputs you can give. Thanks a lot

I dunno about other countries, but in Canada where I lived, we couldn't have our medical exam done until we had the interview package from the consulate. The panel physician just wouldn't do them unless we had the interview letter to show them.

I have both package 3 and 4 for K3 Visa. I have them in 2 zip folders they sent us last january via email with all the forms we needed. But I dont know if they are gonna be the same for Canada. It should be the same though. If you need them send me a PM, butif I were you, I'd contact the embassy/consulate and have them send to you.

I agree with the suggestions that she contact her Embassy to be certain of their rules. In the UK, you must have your case # to put on your medical questionnaire but you don't have to have your interview scheduled before your medical appt. :)

The forms are available under the "Downloads" link at the top of this page - go to Other Immigration Forms & you'll see it. However, it does state that some Embassies use a different form. So I would definitely contact the Embassy to make sure what one they want. Also, they may let her fill it out at the medical appt, so she should check. ;)

Thanks for all those who replied. We figured it out that she can have her medical without the packet since she has an appointment letter that was sent to her husband. I just realized that we do not have the same forms to be used hehe :yes:

YES, SHE CAN GO DIRECTLY TO ST. LUKES MEDIAL CLINIC WITHOUT HE PACKET...ALL SHE NEEDED TO BRING ARE VALID ID, PASSPORT, PHOTOS AND THE FEE.

IT IS ALSO HELPFUL IF SHE KNOWS HER NEW MANILA CASE NUMBER.
