In a population of approximately 15 million, Karachi is known to have one of the highest per capita incidence of oral cancer in the world, say doctors. This is essentially due to the rising consumption of “gutka” ñ the newest addiction which has hit large parts of the city over the past few years.

Unaware of the consequences of this tobacco-based product on oral health, many people consume 8-10 packets daily on an average which is what makes them especially prone to cancer.

Such victims of oral cancer, due to consumption of gutka, usually seek medical help when it is almost too late. They wake up to their addiction when the disease has reached an advanced and incurable stage and that is when they realize their mistake.

Despite repeated government bans on this addictive substance in the past couple of years, the situation has not changed much and gutka continues to be marketed in all parts of the city.

A 65-year-old retired government servant and a resident of Mehmoodabad, Zafar Ali, told The News that in 1992 he was diagnosed with cancer of the oesophagus due to massive consumption of betel nuts and paan.

Ultimately his oesophagus was removed and his digestive system remains upset ever since. He often gets severe cough attacks. But disregarding these problems, he now consumes gutka instead and admits that he is “too addicted to get over this habit.”

Zafar Ali puts this down to the easy availability of gutka. “It’s very difficult for me to stay away,” he confides. In Karachi today, more than 15 types of gutkas of different brands are easily available in the market and some are sold as cheap as Rs3 per serve, making it affordable for all.

Gutka sellers are least bothered whether they are selling to an adult or a child. This product encourages consumption from an early age. Consumers mostly include men, but some young girls and aged women too are equally fond of it.

Shahid Saeed, a shop owner in Saddar, adds: “I can do without food and water but not gutka. Plus we shopkeepers work for long hours and need to munch something every now and then. For this, gutka is just perfect.”

When being interviewed, it was difficult for him to speak because his mouth was full of gutka. Most of the time Shahid Saeed prefers communicating with signs rather than making a verbal attempt so as not to let the gutka spill over.

Gutka itself is a mixture of ground betel nuts (supari), tobacco and flavourings. Rotten betel nut is usually used in its manufacture, despite the fact that it is absolutely unfit for human consumption. The gutka varieties available in the market are coated with non-food colours and sweeteners hence making the fungus present in its ingredients invisible. This fungus releases a toxin that mixes with the saliva and produces cancer. The primary aim of the whole treatment of adding chemicals is to hide the physical condition and odour of the spurious betel nut.

Mohammad Ali, a 38-year-old rickshaw driver and resident of Lyari, is one such example of a gutka addict who is currently admitted in Civil Hospital after being diagnosed with oral cancer.

He disclosed that he had been a regular chewer of gutka for the past five years and was not aware of the severe impacts it has on oral health. “I got into the habit of chewing gutka in the company of my friends. Back then I couldn’t imagine I was inviting my own death,” he says.

Another resident of Sachal Goth said that even though he is well aware that gutka is highly injurious to health and has tried giving up the habit, it seems impossible now because he has been long addicted to it. “My family tried to stop me but I continued to chew behind their back. They are not bothered now,” he smiles.

He added that he had once strived to do away without them but reverted to his old habit after two days. Today, he says, he consumes more gutka than he did six years ago.

It is a pity that not much is being done to raise awareness among the illiterate masses who are attracted to cancerous products like gutka and are not conscious of its ingredients that can prove to be fatal. It is even more unfortunate that those who do manage to make it to the hospital are either too old or too addicted to give it up, which further makes the treatment slow and ineffective.

thats cool, like the "THE NEWS" better. It sound funnier, well atleast some of the stories are written that way. for example, i found this a bit funny

well i think it comes in packets, and you chew it and it all builds up in your mouth. Many of my friends are addicted to that garbage.

GUGTKA like ‘PAN’ comes in wrapping papers I thought it is ban already but lot of folks especially in Karachi takes it.

By the way there is think substitute of Gutka also available in United States in Desi stores

I have seen in Houston and I am pretty sure it is also available in Chicago/NY and LA lots of people take it. No idea how it is available here.

well alot of people also eat just plain tobacco

baba120, 420. with silver leaves. they add flavor to the tobacco. It is ban if you read the article lol, but they still eat in karachi
