So, a)the I693a is not on the uscis website's form list, just I693. Where can I get it?

b ) How are we or the doctor supposed to fill this out? The little info we have on vaccinations is in the medical report which uscis has, not us. She has no records or even dates of her vaccines, although she had them.

c) Does it really HAVE to be filed with the AOS package or can it wait until she has the medical later?

I-693a is no longer valid. You should have a designated civil surgeon fill out the I-693. He should give you a sealed envelope with the I-693 inside.

Also your lady should have the vaccination record [DS 3025] from St. Lukes. Small print at the bottom DS 3025.

Also all should be submitted with the I-485 package.

I wasn't aware the I-693a was not valid. It still refers to it in the instructions for the I-485 when ajusting status. You can get the I-693a form here: Form I-693a

Assuming its still a valid form......If you take it to the civil surgeon with all of your shot records, the doctor will know what to do. Some civil surgeons have the I-693a form already in the office. If your fiance had her shots at St. Lukes when she had her medical, the should have given her a record of what shots she was given. Take that as proof of the immunizations. She also might need some additional shots in the states, (My wife did.) so get those done beforehand and take that record also to the civil surgeon.

Shop around for a civil surgeon. She is a K-1, so she doesn't need a complete medical exam. She only needs to have her shots recorded on the form I-693A. Some civil surgeons will not do it unless you allow them to do a complete medical exam. If you look around, you can usually find one that will just transfer the data and sign the form for a nominal fee. I ended up paying $25.00 to a doctor when others wanted to charge me $300+ for a complete physical exam. Again, it's not needed for the K-1's AOS.

Again, a full medical is not needed for a K-1. It is my understanding that the I-693a is a required form for the K-1 to AOS. If you don't have it, they will ask for it at the interview, or send you and RFE. I have heard of both things occurring to other VJers.

Submit the completed Form I-693, sealed in an envelope from the physician, with your Form I-485 package, to the Service Center or District Office where you are filing your adjustment of status application.

The medical examination must be conducted by a physician who has been designated by USCIS as a civil surgeon. For the current list of civil surgeons in your area, check the Civil Surgeon Locator Tool located under “Related Links” in the upper right corner of this web page. You may also obtain the names and phone numbers of the designated civil surgeons in your area by calling USCIS' National Customer Service Center at 1-800-375-5283. You will be asked to provide your zip code. Please have a pen or pencil ready to write down the list of civil surgeons in your area.

Form I-693 has been revised in accordance with the Centers for Disease Control and Prevention (CDC) updates to the Technical Instructions for the Tuberculosis (TB) Component of the Civil Surgeon Medical Exam and the Technical Instructions for Vaccinations. The revised Technical Instructions for TB include new requirements for the TB component of the medical exam and became effective 05/01/08. The revised Technical Instructions for Vaccinations include new age-specific vaccination requirements and became effective 07/01/08. Note:

I read what you copied from the USCIS site, but I don't see anywhere that it says the I-693a is no longer a valid form. Here is what I read in the instructions for completion of the AOS application, Form I-485, just earlier. I double checked it a few minutes ago for anything that could have changed and it was still the same.

8. Fiancé(e)s.

If you are a K-1 fiancé(e) or K-2 dependent who had a medical examination within the past year as required for the nonimmigrant fiancé(e) visa, you only need to submit a vaccination supplement, not the entire medical report. You may include the vaccination supplement with your adjustment-of-status application.

I'm not trying to be argumentative here as I know things do change quite frequently with the USCIS. I just did not read anywhere in what you posted that the vaccination supplemental form is invalid and that the K-1's have to file a full medical report. If there is something I missed, I apologize in advance.

Haole is correct in that a full medical is not needed by K visa holders, and the supplement for I693a will no longer be filled out by Civil Surgeons. The current procedure is to submit an I693 with a completed Part 1 to a civil surgeon. The civil surgeon will fill out the vaccination section of part 2 (based on vaccination records, some may still have to be administered), and part 5 (civil surgeon's ceritification). This will be given in a sealed envelop to submit with AOS.

I had only read the instructions for the I-485 and it seems the instructions for both forms give conflicting information in my opinion. Since the I-693 doesn't appear in the general list of forms on the USCIS.gov website and with what you pointed out in the other form, I will agree you are probably correct.

We filed for AOS in July of last year and we got the form I-693a from the same place I posted because it did not appear in the USCIS form files listing then either. We used it for our AOS, based on the I-485 instructions, and never had a problem with it. No RFE's or questions at the interview.

Heck no offense taken AT ALL. When USCIS makes changes it takes qiute awhile for things to get straightened out. They don't take the trouble to remove older sites and

wrong info still pops up.

The change is very recent. I693 was changed on 06/05/2008, and they only just stopped using the I693a. We filed this past May for AOS and the CS gave us I693a in a sealed envelope and a copy for our records as they had in the past and when you filed.

Vaccination requirements changed for AOS applications after August 1, 2008; and hence the change in "vaccination supplment" from the I693a to I693 with just the vaccination sections filled. There has been much discussion in the AOS forum about the new vaccination requirements (and the new TB test requirement for any full medical done after August 1, 2008).

Form I693 is on the list of forms from USCIS.gov under the "Immigration Forms" tab. The I693a is not and never was. CDC.gov was the place to go.

Basically all you need to do is fill in part 1, part 2 number 5 and part 5 of the I-693. Vaccinations is the focus.

When was your medical? Here's a link to the new info in regards to exams... Great link that explains this stuff pretty clearly... what's necessary in terms of vaccines and TB test requirement since the form has changed.

6/13/08 Applied for SSN for David ----- TRIP 1; 6/23/08 Went back to SSA 'cuz the person didn't pc all documents (rec'd letter from them) ----- TRIP 2

7/2/08 Received David's health insurance cards! Yay!

7/3/08 Went back to SSA; they said come back 7/9 if he still hasn't received it ----- TRIP 3

7/8/08 Got list of vaccines he still needed from CS by fax/phone; tested for immunity to MMR (only vaccine still needed) at our family dr
